Quiz : Abdominal pain in a patient with Paroxysmal Nocturnal Hemoglobinuria
A 42-year old man presented to emergency room with abdominal pain, nausea and vomiting for the last 4 days. He was diagnosed as Paroxysmal Nocturnal Hemo- globinuria (PNH) a year before and he had thrombocyto- penia due to PNH. Physical examination revealed abdominal distention and abdominal sensitivity with re- bound tenderness. His international normalized ratio (INR) was 1.12 and platelet count was 14000/mm3. What would you recommend for the evaluation of abdominal pain and what is your diagnosis ?
